*For dollar amounts from $25 to $250, there is a processing fee of $5.95. For dollar amounts from $251 to $500, there is a processing fee of $7.95. Cards are valid for one year from date of issue. Six months from date of issue, a recurring monthly maintenance fee of $2.50 will be charged to the balance. Cards can be purchased by calling 1-866-874-9029. There is an additional fee of $4 per card for orders placed by telephone.
